Session Overview

HI! We are a newer nonprofit given our first opportunity for a breakout session at The Rare Fair in Sept. We also paid for a booth. This coming Friday, 24th they are hosting an open house and Duke University Hospital among others will be there. They said if I could get them marketing materials by 23rd they would print!

We have five modules and first two have been beta tested. We have data and a typed letter by the Social Worker at the High School. We also have a few handwritten from participants between 17 & 18 years who attended Your Mental Health Toolbox module.
There is a podcast in my linkedin feed of both me and Ivy our Pediatric NP if you would like to hear our voices and how we speak without notes!

The Healthcare Navigation Project is an independent 501(c)(3) intentionally designed to disrupt our complex healthcare system with a health literacy and mental health life-skills program for ALL youth transitioning to adult care (HCT). This includes rare or chronic disease, underserved communities, or those with educational disabilities.
