My goal for this Taproot Project is to create a well-designed client survey that captures meaningful feedback while remaining simple and accessible for the families we serve. We are looking for support in developing two survey formats: one for initial intake and another for six-month follow up. The surveys will need to be available in both digital and paper form, written at a maximum seventh-grade reading level, and translated into English, Spanish, and French. The volunteer will help us ensure the questions gather essential demographic information without being invasive, while avoiding personal identifiers such as names or addresses. This project will give us a reliable tool to measure program outcomes and community needs more effectively.
This project will directly strengthen our mission by allowing us to better understand and respond to the needs of families in our programs. A clear and accessible survey will give us insight into the effectiveness of our services and highlight areas for growth. By capturing this feedback at intake and again after six months, we will be able to demonstrate impact to funders, refine our programming, and ensure families feel heard in the process. Volunteers who partner with us will know their work is helping to improve real-world outcomes for families in need and equipping our organization with data that drives long-term community benefit.
My team and I have prepared for this project by identifying the key data we want to capture and clarifying how the results will be used to improve our impact.We are ready to provide examples of previous surveys and internal data priorities. We have also secured organizational buy-in, so once the surveys are finalized, our staff can immediately implement them in both paper and digital formats. This preparation ensures that the volunteer’s work will move quickly into practice, not remain on a shelf.
