There is a grant/sponsorship given out by Hartford Healthcare here in Connecticut and we hope to apply to use as a pilot project for Child and Family agency who has 21 School Based Health Centers. I really could use to talk through to make sure we are covering everything we need before we submit!

Organization Mission

The Healthcare Navigation Project is an independent 501(c)(3) intentionally designed to disrupt our complex healthcare system with a health literacy and mental health life-skills program for ALL youth transitioning to adult care (HCT), and everyone else. This includes rare or chronic disease, underserved communities, or those with educational disabilities. We bridge the gap in healthcare by empowering all people to learn to actually use healthcare and self-advocate before they need it instead of after when it's too late. We empower people.
